1. Purpose of This Document
Before receiving any IV hydration, vitamin shot, peptide therapy, NAD+ therapy, or weight-loss injection service at The Hydro Fit Lounge, clients are asked to review and sign a location-specific informed consent form. This page summarizes the general purpose and content of that process; it does not replace the signed consent form you will complete in person or through our online intake system prior to treatment.
2. Services Covered
Our informed consent process applies to all injectable and IV-based services, including but not limited to: IV drip treatments, vitamin B12/biotin/amino acid shots, peptide therapies (such as BPC-157, CJC-1295/Ipamorelin, GHK-Cu, Kisspeptin, KPV, MOTS-c, PT-141, Selank, Semax, TB-500, and related stacks), NAD+ therapy, and GLP-1 weight-loss medications (such as Tirzepatide).
3. General Risks & Side Effects
As with any injectable or IV therapy, potential risks and side effects may include but are not limited to: bruising, discomfort, or swelling at the injection/IV site; infection; allergic reaction; nausea, headache, or lightheadedness; fluctuations in blood pressure; and, for weight-loss and certain peptide therapies, gastrointestinal side effects, changes in appetite, or other medication-specific effects. Your consent form will describe the specific risks associated with the exact service you choose, and our staff will answer any questions before you begin treatment.
4. Voluntary Participation
Participation in any service at The Hydro Fit Lounge is entirely voluntary. You will only receive services after confirming that you understand the nature of the treatment, its potential risks and benefits, and any reasonable alternatives, and after your questions have been answered to your satisfaction.
5. Right to Refuse or Withdraw
You have the right to decline any recommended service, or to stop a treatment in progress, at any time, without penalty other than payment for services already rendered up to that point.
6. Client Acknowledgment
By signing the consent form provided at your appointment, you acknowledge that you have disclosed relevant medical history, understand the treatment being provided, have had the opportunity to ask questions, and voluntarily agree to proceed.
