This notice describes our privacy practices and your rights regarding protected health information ("PHI") in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
2. What Is Protected Health Information?
PHI is health information — such as intake forms, medical history, current medications, allergies, lab results, and treatment records — that can be used to identify you and relates to your past, present, or future physical or mental health condition, the care you receive, or payment for that care.
3. How We Use and Disclose Your Health Information
We may use and disclose your PHI for the following purposes without additional authorization:
- Treatment — to provide, coordinate, or manage your care, including sharing relevant information with our supervising provider or clinical staff involved in your treatment.
- Payment — to bill for and collect payment for services rendered.
- Healthcare operations — for quality assurance, staff training, and business management activities related to your care.
Other uses and disclosures — such as for marketing purposes, or sharing with third parties not involved in your care — will only be made with your written authorization, which you may revoke at any time.
We may also be required to disclose PHI without authorization where required by law, such as in response to a court order, public health reporting obligations, or to prevent a serious threat to health or safety.

- Request access to and a copy of your health records.
- Request corrections to your health information if you believe it is inaccurate or incomplete.
- Request a list of certain disclosures we have made of your PHI.
